This form collects additional details we need to set up your profile in our system and ensure we have everything ready to prepare your return accurately and efficiently during tax season.

In addition, please provide copies of your last two years of tax returns. Reviewing these helps us understand your tax history and identify any opportunities or issues that may affect your current or future filings.
